Nutrien’s New Safety Program

Presentation Overview

A Case Study in the lead up and ultimate failure of the radiant tubes in a Primary Reformer for a 1960's vintage ammonia plant. The extent of the physical failure will be described in addition to the underlying root and contributing causes. This incident will also depict how this was a "ground zero" type event for building a PSM program that is focused on prevention, discipline and integrity.
