A Case Study in the lead up and ultimate failure of the radiant tubes in a Primary Reformer for a 1960's vintage ammonia plant. The extent of the physical failure will be described in addition to the underlying root and contributing causes. This incident will also depict how this was a "ground zero" type event for building a PSM program that is focused on prevention, discipline and integrity.

Chris has a Bachelors of Applied Science (BASc) in Chemical Engineering from the University of British Columbia. He has spent the last 14 years working at a manufacturing facility supporting the production of Nitrogen and Phosphate based fertilizers. In the various roles he has worked at the Redwater Fertilizer Operations facility he has supported both day-to-day operations and long-term capital project development and execution. In 2011, Chris officially began his foray into Process Safety Management (PSM) working to further reduce the risks associated with fertilizer manufacturing by implementing strategic risk reduction projects. He would later grow his role and become the focal point for the PSM program responsible for managing and updating the program for the Redwater site - Canada's largest manufacturing site. Today Chris and his team support the mission to achieve safe, reliable production via execution of the principles and processes within a comprehensive PSM program.
